Two new immunizations requirements for incoming 6th
graders.
All sixth grade students are required to have two new immunizations prior to the start of the sixth grade school year.
The following are the school immunization requirements of the New Jersey State Department of Health for all children enrolled
in school. If your child’s immunizations are incomplete, please make an appointment with the Pediatrician and forward
the school a copy before September of the 2011-2012 school year.

Physical Exams:
The new regulation, N.J.A.C. 6A:12-22 states "The primary responsibility for the health of a child rests with the Parent/Guardian and their family physician". When you take your child for their annual physical exam - take a copy of the following form and have the doctor complete the following form: Universal Child Health Record
N.J.A.C 6A:16:2.2(h) specifies Athletic Pre-Participation Physical Requirements. Each candidate for a school athletic squad or team is to be examined within 365 days prior to the first practice session. Students may use their private medical home or the school physician. The athletic physicals form must be completed and be on file in the nurse's office prior to participation.
The nurse's office is equipped to render immediate care to pupils in
case of illness or accident. If an accident or sudden illness occurs, first
aid will be administered and the child's parents will be notified. No care
beyond first aid (defined as immediate and temporary) will be given. If
parents cannot be reached and the emergency warrants the assistance of a
physician, our school doctor will be called. If unavailable, an ambulance will
be called. All parents are requested to update emergency phone numbers during
the school year, if any changes occur.
Medication Policy
Parents are required to bring all medications to school. Students are
never permitted to carry medications on the school buses or in the school.
All medications (prescription and over the counter) require a doctor's note
and completed parent
permission form. All asthma students are permitted to carry their inhalers on
the bus and in school after providing the nurse with a doctor's note and
parent permission form. Any student who carries medication to school or on
their person will be subject to the school discipline policy.
Food allergy/anaphylaxis action plan
Too sick for school?
You don't want your child to miss school; but do you want to send a sick child to school and endanger others as well? No. When should your child stay home? Here are a few guidelines you may wish to follow:
Cold Symptoms- If your child is continually coughing, congested and running a fever (100.0 F and above) keep them home and check with your doctor. Your child must be fever-free for 24 hours prior to returning to school.
Diarrhea and Vomiting- If your child is vomiting and/or has diarrhea keep them home until the illness passes and consult your doctor.
Strep Throat/Scarlet Fever- These are two highly contagious conditions caused by a streptococcal (bacterial) infection. They usually start with a sore throat and a high fever. A child with either strep throat or scarlet fever must be kept home and treated with an antibiotic as prescribed by a doctor. A doctor's note is required for your child to return to school (usually 24-48 hours on the medication).
Rash- If your child has a rash and/or is itching (with or without a fever) do not send your child to school until they are evaluated by a doctor. Please get a "return to school" note stating when your child is allowed back in school.
Conjunctivitis (Pink Eye)- If your child complains of eye discomfort with redness or a discharge do not send to school until evaluated by a doctor for "pink eye". This is highly contagious and usually requires antibiotic eye drops. This condition also requires a doctor's note to return to school.
Fever- A fever is an important symptom. School policy requires children to be sent home with temperatures above 100.0 F. Keep children home during the course of a fever and for an additional 24 hours after their temperature has been normal.
Chicken Pox- This viral illness is contagious. Please let the nurse know if your child has been diagnosed as having chicken pox. Students do not require a doctor's note to return to school but there must be no blisters or drainage present before returning to school.
