FYI: School doctor will be in on September 8th & 15th
All Davies student athletes are required to fill out the following paperwork in order tryout and compete for the schools interscholastic teams. Students cannot participate if all forms are not filled out before the start of a given season. (see below)
***First two forms need to be completed and turned in before a student can tryout/participate in interscholastic sports***
2- Annual Athletic Pre-Participation Physical Examination Forms
(Physicals are good for 1 year from the date of physical and must be on file with the school nurse)
3 - Student Athlete Contract (Completed once a member of a team)
Seasons Begin:
Fall Sports - 2nd week of September
Winter Sports- Mid November
Spring Sports - 1st week of March
